CareFirst’s Patient-Centered Medical Home (PCMH) Program provides PCPs with exclusive access to resources like electronic medical records and a large network of specialized nurses to help them better coordinate your overall health.
PCMH PCPs help guide all your care including specialists, lab work and prescriptions and focus on preventing problems before they begin. Using your PCMH PCP's office as your first contact or "home base" for most medical needs ensures you get the care you need when you need it, leading to better health, better communication and better results.
PCMH PCPs play a huge role in keeping you healthy for the long run. In addition, selecting a PCMH PCP puts you one step closer to earning your Blue Rewards.
To find a PCMH PCP, look for the PCMH logo when searching for primary care providers in our Provider Directory or log in to My Account and click Select/Change PCP under Quick Links.
